A Clinical Characteristics of the Gastric Carcinoma According to Histological type


Abstract
Because of variable histopathological finding in gastric carcinoma, this study was done to define clinical features and characteristics for the different pathological type of gastric carcinoma.
From January 1989 to December 1991, 213 patients who underwent operative procedure for gastric carcinoma were classifed according to Nakamura's classification and compared with age, sex, tumor location, macroscopic finding, lymph node metastasis,
distant metastasis, and operative procedure.
Two hundred thirteen patients were identified and classifed into two groups: 96 patients (41.1%) had a differentiated adenocarcinoma and 117(58.9%) patients had an undifferentiated adenocarcinoma. In younger age than 40 years, the
undifferentiated
type
were more frequent than differentiated type by the ratio of 1: 1.87, but tumor location, lymph node metastasis, and operative procedure showed no significant difference between differentiated type and undifferentiated type. The ulcerative type
and
TNM
stage II were more common in the differentiated type and TNM stage III was more common in the undifferentiated type(P<0.05). The patients with differentiated type were more common to serosae invasion(P<0.01), but invasion to neighboring
structures
were
more frequent in the undifferentiated type.
In the distant metastasis, liver metastasis was more more frequent in the differentiated type, statistically significant(P<0.05), but peritoneal dissemination showed no difference be ween differentiated type and undifferentiated type.

In TNM stage IV gastric cancer, 1 year survival rates were 32.1% in differentiated type and 31.4% in undifrerentiated one. The median survival times were 10 months in the former and 9 months in the latter. Neither 1 year survival rate nor median
survival times had statistical differences.
